For the safety of our staff and our clients, we reserve the right to cancel or reschedule service for anyone who might pose a health risk at this time. To aid us in that effort, we ask you to complete the following form before your appointment. If you are unable to confirm any of the following questions, please contact us immediately to discuss your situation. Thank You! In addition, please be advised that no one will be permitted to wait in the barbershop while waiting for guests to get haircuts and that we reserve the right to require guests: 1) to wear masks, 2) have their temperature checked upon entry and 3) wait outside until the chair is open for them

Please confirm that you have not experienced any of the following symptoms in the last 14 days: Fever, Shortness of breath, new or worsening cough, runny or congested nose(not related to seasonal allergies or known condition), chills, muscle pain, headache, sore throat, difficulty swallowing, new loss of taste of smell, digestive issues or diarrhea, unusual fatigue, falling down often, sluggishness or lack of appetite( if young child)
